I can't see buck teeth interfering with a ballgag since any ballgag that doesn't fit behind your front teeth is probably too big for your mouth.
As for how effective it is, it depends on how much you can open your mouth. A standard ballgag isn't too big and won't do much to interfere with speech. A larger one will more significantly reduce your ability to talk, but the larger it is the more your jaw will ache while it's in there unless you're used to it. Still, as long as you can get air in/out your mouth and open your jaw around the ball, you can still make some significant noise. The best ballgags would be those that have harnesses that hold go around your head and hold it in place along with a mouth covering that helps prevent noise.
If you're just looking for noise reduction, a sturdy tape gag with stuffing or an inflatable gag is better, ballgags are mainly used for the look, garble speech and how they make people drool.
