First, not everyone has the same reaction to the same stimulus. Some people are in agony from dental work even with anaesthetic. Others can tolerate drilling without any novocain. Just because you can't tolerate something you see doesn't mean that the person experiencing it is suffering as much as you would in that person's place. And it's not at all unusual for person A to be able to tolerate something B can't stand, while B has no problem with a different thing that A can't take. I should also point out that some implements such as floggers look and sound a whole lot worse than they really are. They can make a frighteningly loud sound while stinging far less than a bare-handing spanking.
Some people enjoy some level of pain for its own sake - as tony2 noted, it releases endorphins. But that's not the only possible reason for introducing pain into a TUG. Some people role-play an interrogation. Pain makes the game more real. Some people enjoy not the pain itself, but rather the psychological challenge of standing up it.
