I have seen it done a different way, involving (this will sound weird but it can be done) a bucket tied over a pulley at the other end (with the 'victim" unable to reach the rope to slacken pressure) and then the bucket is slowly (and very carefully) loaded with water to increase the pull on the testicles so that sudden and potentially crippling increases in pressure could not happen but only gradual increases. The victim could then cry uncle well before things got out of hand this way.
In general however, I do NOT recommmend this type of play unless you really know what you are doing. Even a small mistake could be worse than just painful. If you feel you have to do it, go very, very carefully. You are NOT going to want to explain to a medic or doctor (or an insurance company, worse yet) how an injury there came about.
Most policies do not cover self-inflicted or assisted-inflicted injuries. And NEVER be alone while in this situation - EVER! This is as important as it is with gags and stringent tie-ups!
Sorry if you know this already, but I think such warnings cannot be over-stressed.
